Additional Requirements: We will email or fax a form to you that you will need to have notarized then fax to our office to complete your order (this is a state requirement). We cannot obtain birth certificates for the following counties: Kings County, Calaveras County, Colusa, Amador, Lake, Del Norte, Yolo, Siskiyou and San Francisco.
