Additional Requirements: If the applicant is a minor then we will need a copy of one of the parents drivers license or state issued identification card. If you do not have one of the previous you will need to provide at least 2 of the following: work ID card, paycheck stub, W-2, voter registration card, Social Security card, DD 214, motor vehicle registration or title, probation documents, marriage license, Medicaid card, Social Services card, Department of Corrections ID card.
