Additional Requirements: A copy of one of the following: Drivers License or state issued Identification Card. If you cannot provide one of these you can provide a copy of 2 of the following, these must have your current address on them: Social Security Card, Bank statement, utility bill, doctor/medical/hospital bill, car registration or title with current name, valid health insurance card, income tax with current name, W-2 from present year with current name, US Voters Registration card or current pay stub which must include name and address of the employer and Social Security number.

We must have the full social security number of the applicant (if the applicant is a child then it will be one of the parents SS #) on our order form, this is a state requirement. We can only ship the birth certificate to the credit card billing address (where you get your bill every month), this is a Kansas state law.
