Additional Requirements: We MUST have a uploaded copy of your State issued photo ID on the order form. You can also take a picture of your ID with your cell phone and upload that on the order form or email it to us. It speeds up your order to upload the copy of your ID.

We MUST have the birth certificate holder or a parent's (if the parent is ordering) Social Security number and date of birth on the order form. The state of Texas requires this.
